Aging Family Solutions offers a free 30 minute care management consultation to start the "listening" phase of our program. Afterwards, we can schedule any or all of the below services:
Develop individually tailored care plan based on overcoming the senior's specific challenges either in the home environment or community setting, by providing a unique and appropriate level of support in order to achieve optimal outcomes.
Typically takes 3-5 hours depending on medical complexity, and should be completed twice/year or at any significant health status change.
Determine if home care, home health, rehab therapy, nurse care or other services are needed, which ones are the best fit based on their specific needs, insurance and finances, vett the support, manage the support, communicate the support.
Develop a comprehensive plan to cover all phases of the move:
- Determine which level of care is needed
- Schedule tours at top 3 best fit communities and accompany
- Negotiate rates when possible
- Assist with all paperwork
- Facilitate the move
- Set up the new living space
- Ensure seamless transition
- Monitor adjustment
- Provide regular check ins, and follow up with family
*No charge for existing clients

- Well Checks (blood pressure, med mgt)
- Mail Sort/Bill Pay

*2 hour minimum
*Cancellation fee with less than 24 hour notice = Full charge.
*Rush job within 24 hour notice + $25.
*Holiday visits + $25.
*24 hour on call standby respite + $575/24 hrs
